3 simple steps!

1. REGISTRATION

Download and fill in the following documents:

2. COMPANY INFORMATION

Gather all the required supporting documents in soft copy:
  • Download and fill in the Letter of Consent
  • Main Service Order Form - signed by an authorised signatory
  • Service Order Form - signed by an authorised signatory
  • Application Service Provider (ASP) License (with valid effective date)
  • Certified true copy of Certificate of Registration (Form 9*) or Certificate on Change of Company Name (Form 13*)
  • Certified true copy of Return of Allotment of Shares (Form 24*) with at least RM100,000 of paid-up capital
  • Certified true copy of Registered Office (Form 44*)
  • Certified true copy of Directors Particulars (Form 49*)
  • Certified true copy of Memorandum And Articles Of Association (M.A.A) - please ensure that the MAA allows the company, at the minimum, “to carry out business as an Information Technology provider” or in similar words
  • Company’s latest 3 months bank account statement (please cross out the detailed transactions)
  • Note:
  • All Certified true copies are required to be dated within the last 3 months.
  • *Or its equivalent document according to Companies Act 2016.
